It is not required to have a High School Diploma or GED to enroll, however, if you do then I know that you have the English proficiency to benefit (successfully pass) this training course. The other option is to take the Ability-to-Benefit Test if you are do not have a High School Diploma or GED from an English speaking country.

Please select one option:
I have a High School Diploma from an English-speaking country and will provide documentation on day one of the program.
I have a GED from an English-speaking country and will provide documentation on day one of the program.
I would like to take the Ability-to-Benefit Test

If you choose to take the Ability-to-Benefit Test, we will contact you regarding scheduling and testing requirements.

Tuberculosis (TB) Screening
I have completed a TB skin test or blood test and will provide documentation no later than 1 week before the start of the program.
I understand that it is up to me to receive the TB testing before the program begins and provide documentation when the TB testing is completed, before the start of clinical.

Tuberculosis (TB) screening is required before participating in clinical training. If you have already completed your TB test within the past 12 months, bring your documentation to class on day 1. If you have not yet completed TB testing, it is up to you to be tested and provide documentation of the negative TB test no later than one week before the start of the program.

Influenza Vaccine
I have received the current seasonal influenza vaccine. I can provide documentation showing that I received the influenza vaccine for the current flu season.
I have not yet received the current seasonal influenza vaccine. I understand that it is my responsibility to obtain the current seasonal influenza vaccine and provide documentation no later than one week before the start of clinical. I understand that influenza vaccines are generally recommended beginning in the fall, with vaccination ideally occurring in September or October.
Choose this option if the following applies: It is outside of Influenza season, or you decline receiving the Influenza vaccination and agree to wear a surgical mask at all times while in the clinical facility during influenza season (October through April).

Students participating in clinical experiences may be required to provide documentation of influenza vaccination in accordance with the requirements of the clinical facility.
